Sometimes a machine or process problem we have just solved comes back and we have to spend more time and resources to repair the machine or process. Why does this happen?
Often, a problem comes back because we were not able to address the true cause of the problem and instead only dealt with the symptoms. As my daughter who is doctor would point out, most cold medicines only mask the symptoms. The meds may help you feel better, but only rest, hydration and time will actually cure the cold.
Problems in operations can be very complex and difficult to analyze. This is where RCA becomes very useful.
Root Cause Analysis (RCA) is a very effective technique for discovering the root cause of problems that we encounter in our day to day operations.

Often, the simple problem solving techniques we may have learned are not enough to identify the real root cause of the problem. Perhaps the root cause we chose was really the result of guesses, conjectures or gut-feel. This is often referred to as the “touchy-feely” approach and is based more on your intuition, bias and simple logic rather than on hard data.

The RCA process provides us with a methodology and a set of tools to analyze the problem and the data to get to the true cause of a problem and develop countermeasures to prevent the recurrence of the problem.
