Failure modes and effects analysis (FMEA) is a disciplined approach that you can use to greatly improve your operations, products, services, prevent accidents, and unintended failures of your operations. FMEA is used to identify possible failures of a product or service before they occur and then help you quantify the the risk of that failure occurring. With this knowledge, you can then incorporate countermeasures or process improvements that will reduce the risk of the harmful event occurring in the first place.

The ISO 9001:2015 revision makes “risk based thinking” more explicit and builds it into the whole management system.
